COVID-19 Vaccination Requirement—Student Process

As announced in May, all UO students and employees are required to be fully vaccinated against COVID-19 prior to the start of the fall academic term. This public health measure at the UO will help protect our community by reducing the potential for serious illness, hospitalization, or death. It also will limit campus disruptions and allow us to resume in-person instruction and activities. Students who do not complete either action for fall term will be subject to a registration hold and subject to discipline under the student conduct code.

Your vaccination status will be kept confidential. The forms and processes for submitting information to comply with the vaccination requirement are secure and HIPAA compliant.

Complete the Vaccination Requirement Process

To meet the UO COVID-19 vaccination requirement, UO students are required to attest to being fully vaccinated or request an exemption. Exemptions are allowed for medical, religious, philosophical, or other non-medical reasons.

Steps to attest being fully vaccinated for COVID-19:

  • Students should log into MyUO Health to complete their COVID-19 vaccination requirements.
  • Once logged in, go to the “immunization tab.”
  • Students are considered fully vaccinated according to CDC guidelines. Those who intend to be vaccinated, are in the process of receiving a two-dose vaccine, or within 14 days of completing the vaccination process, should wait to submit the information until they reach fully-vaccinated status.
  • UO reserves the right to request additional proof of vaccination if necessary.
  • The UO will accept any COVID-19 vaccine that has been approved by the World Health Organization.

Request an exemption:

Students who request an exemption are required to take part in weekly COVID-19 testing through the UO Monitoring and Assessment Program, among other safety measures. These measures may include isolating or quarantining if you are exposed to or contract COVID-19, testing to participate in certain activities and/or being excluded from certain activities.

Request COVID-19 Vaccine Exemption

  • Students requesting an exemption should login to MyUOHealth, select the “forms” tab, look for "Other forms" section, and select the COVID-19 forms.
  • Review of an Oregon Health Authority COVID-19 vaccine exemption education module must be completed; and student must acknowledge that they will be expected to continue taking additional safety measures as required for unvaccinated individuals.


The deadlines for complying with the UO COVID-19 vaccination requirement regulation are:

  • Law School JD and LLM students: August 13
  • UO residence hall students prior to assigned move-in date: no later than September 14
  • All other students: September 27

Vaccine Requirement Information

  • The online COVID-19 vaccination requirement requires a student to be fully-vaccinated according to the following Centers for Disease Control and Prevention guidelines:
      • 14 days after receiving a one-dose COVID-19 vaccine.
      • 14 days after the second dose of a two-dose COVID-19 vaccine.
      • Vaccinations administered outside of the US must be “EOI accepted” by the World Health Association and have a fully completed series according to the manufacturer of the vaccine.

Due to the 14 days required to obtain full-vaccinated status, students who have not yet been vaccinated or have begun the vaccination process should carefully consider their options and account for the timelines required for available vaccine options.
